By registering for and/or participating in Urban Community Connections’ Mom & Baby Fest, I hereby grant Urban Community Connections (UCC), its partners, sponsors, funders, and authorized representatives the irrevocable right and permission to photograph, record, film, and otherwise capture my image, likeness, voice, and/or participation in any format (including but not limited to photography, video, audio, and digital media).

I understand and agree that these images and recordings may be used for program documentation, reporting, evaluation, grant compliance, storytelling, marketing, fundraising, education, and promotional purposes, including but not limited to websites, social media, annual reports, grant reports, newsletters, presentations, print materials, digital publications, and other communications aligned with UCC’s mission of Connecting Communities to Vital Resources Paving the Way for a Better Tomorrow.

I further waive any right to inspect or approve the finished product or the use to which it may be applied, and I release Urban Community Connections, its board, staff, volunteers, sponsors, funders, and partners from any claims, demands, or liability arising out of or related to the use of such media.

This release applies to all participants, including adults, children, and families, and is binding upon registrants, attendees, and their legal guardians.

Participation in this event constitutes acknowledgment and acceptance of this release.

1. Assumption of Risk

I understand that participation in event activities, including but not limited to bounce houses, inflatables, soft-play areas, games, interactive experiences, demonstrations, and activation zones, involves inherent risks that may result in injury, illness, property damage, or other harm. I voluntarily assume all such risks, whether known or unknown, foreseeable or unforeseeable, associated with participation in the event and related activities.

2. Release of Liability

I hereby waive, release, and discharge Urban Community Connections (UCC), its officers, directors, employees, volunteers, contractors, agents, sponsors, partners, vendors, funders, host sites, and affiliated organizations from any and all claims, demands, actions, causes of action, damages, costs, or liabilities of any kind arising out of or related to participation in the event, including but not limited to personal injury, bodily harm, illness, death, emotional distress, or property damage, whether caused by negligence or otherwise, to the fullest extent permitted by law.

3. Indemnification

I agree to indemnify and hold harmless Urban Community Connections and all released parties from any claims, liabilities, damages, losses, or expenses (including reasonable attorney’s fees) arising from my participation or the participation of any minor(s) in my care.

4. Medical Authorization

In the event of injury or medical emergency, I authorize Urban Community Connections and its representatives to secure emergency medical treatment for myself and/or any minor(s) in my care, and I agree to be financially responsible for any medical services provided.

5. Parental/Guardian Consent for Minors

If I am the parent or legal guardian of a minor participant, I consent to their participation in all event activities and agree to the terms of this waiver on their behalf. I accept full responsibility for supervising any minor(s) in my care during the Event.

6. Binding Agreement

I understand that this waiver is intended to be as broad and inclusive as permitted by applicable law and is binding upon myself, my heirs, assigns, and legal representatives.

By my registration, check-in, or participation in this event, I affirm that I have read, understand, and voluntarily agree to this Liability Waiver, Assumption of Risk & Release of Claims.
